At Cirrus House we believe consumers should know there rights and responsibilities for various services we have to offer.
Cirrus House Inc. Program(s) Rights and Responsibilities.
YOU HAVE THE RIGHT:
To receive services, regardless of race, ethnicity, age, color, religion, gender, national origin, sexual orientation, physical or mental handicap, developmental disability, financial difficulties/inability to pay, or other identified groups not specified.
To be treated with respect & dignity by staff & others enrolled in services. Cirrus House Inc. is intended to be a safe environment free from abuse including, but not limited to: exploitation, retaliation, humiliation, neglect, etc.
To make sure you are enrolled in the best program based on your personal strengths, needs and goals.
To be told of your diagnosis, treatment options and other potential programs which best fit your strengths & goals.
To be educated on the details of the program(s) in which you are enrolled.
To have all your information kept private, with a few exceptions; (refer to confidentiality & informed consent).
To have access to your records including treatment plan, notes & evaluations within two business days of a request. Cirrus House Inc. reserves the right to charge a fee (e.g. paper, ink, etc.) when applicable.
To be told when and why your case is being closed/discharged or told why you do not qualify for services.
To know how much treatment costs (if applicable). Cirrus House Inc. will provide the sliding fee scale to all qualifying individuals.
To inform and/or ask staff about help with a grievance. Refer to the member handbook for more information.
To seek services at an organization which better serves your needs be it mental health, physical health, or financial stability.
To not be photographed or recorded while at Cirrus House Inc. without your consent. However, there are cameras in public areas for safety.
To be safe. In the event you may hurt yourself and/or others and are no longer capable of taking care of your financial/medical needs, staff may contact Adult Protective Services to ensure the protection of yourself and others’.
YOU HAVE THE RESPONSIBILITY:
To be part of the treatment planning process/updates to ensure your strengths are utilized and person goals are met.
To respect the confidentiality of PHI of other participants enrolled in services (and staff) as yours is respected.
To exhibit respectful behavior. Violence and/or combative behavior towards staff or other’s enrolled in services is not tolerated at Cirrus House Inc. Emergency intervention may occur when such behavior is observed.
To be respectful with your words. Offensive language is not allowed. The communication between anyone enrolled in services and staff must be with dignity and respect.
To ensure your services are paid for in full (if applicable).
To fully participate and engage with the program to which you have been enrolled.
To keep all scheduled appointments. All staff work with multiple consumers and, therefore, you must keep appointments. Cancellations or rescheduled appointments must be communicated to staff 24 hours in advance or you may lose the privilege to participate in the program. If I miss multiple appointments with Cirrus House Inc. and fail to cancel or reschedule in advance, Staff will attempt to contact me.
To be appropriate and professional. Sexual advancements towards any staff will not be tolerated and will result in dismissal from the program.
To restrain and contain your pet(s). Your service pet is your responsibility and must be restrained. Additionally, service pets must have all proper documentation.
